In the first half of the 20th century, two treatment approaches, both surgical, were available for BPH--open prostatectomy and TURP. With improvements such as fiber-optic lighting and the Hopkins rod lens wide-angle system 35, TURP became the preferred treatment, not only for severe conditions secondary to BPH, such as urinary retention and hydronephrosis, but for less severe yet bothersome symptoms, such as urgency and frequency 36. Currently, at the beginning of this new century, TURP is still the benchmark therapy for BPH36. Nonetheless, there are now many acceptable alternatives to TURP that are less costly per treatment episode, that require less time to produce positive outcomes, and that have less associated morbidity. These alternatives, which are listed in Tables 1.1 and 1.2, include medical therapies, such as alpha-adrenergic blockers, the minimally invasive options, such as transurethral microwave thermotherapy TUMT ; , and the commonly utilized investigational treatments, offer patients a range of choices based on the degree of their symptoms and the presence of uncommon or serious complications. Dysfunctions. A distinction often misunderstood is that dysfunction starts out of the immune system, not out of casein, gluten or other metabolic sensitivities. Children with autism have a lot of metabolic abnormalities, but that is a result of the problems with their immune systems. If a metabolic dysfunction were the cause of a disorder, correcting it would eliminate the disease. If casein or gluten caused autism, eliminating them from the child's diet would cure them, but that does not work. If metabolic dysfunction is a secondary factor of autism, you rarely, if ever, are going to have a patient recover, by treating the "secondary" rather than "primary" problem. Similarly, if it were true that adults with chronic fatigue have a metabolic defect, how come most of them were normal and generally high functioning for years? In medical school I was taught to, get to the reason, and to get to what's underneath it. It's important not to just treat a symptom, or what appears to be on the "surface, " but rather it is necessary to treat what is causing the problem. Medical Treatments Most of the children I see have healthy bodies with reactive and volatile immune systems. The first step, is to check functioning of various systems in the body. Unless another "medical" problem is found, the immune system is what is creating the misbalance dysfunction in the brain. Unfortunately, new, potentially safe immune modulators steroids, IVGG, are old immune modulators, neither generally safe or effective with this type of immune disorder ; are not yet available. Until these immune modulating drugs are scientifically tested in controlled studies, the way to help these children must focus on an overall approach using efforts steps and medicines available now. By the time a child is referred to my office, their immune systems have not been functioning well for a very long time. This dysfunctional process did not occur overnight and it takes time to "cool" down help "normalize" the body and the immune system. The closer you can bring the body towards normal, the better the chance that the body may shut off this reactive and dysfunctional immune system. Shane Murphy likes to listen to his inner voice. When he hears the words "Just do it", he responds by seeking new challenges, exploring different environments and breaking a few rules about "how a person living with Parkinson's should behave". The mantra has resulted in diverse personal and professional interests, from travelling to Thailand for some R & R, doing the grapevine underwater as an aerobics instructor, and getting down and dirty as an Army Reservist. His best friend for the past decade or so is year old Zac, a white Appaloosa that he visits twice a week, and with whom he joins Sea Mist, the Otways Organized Trail Riders for some time out on group rides. Shane finds riding helps him maintain and develop greater balance and strength, and encourages him to use both sides of his body equally as he manoeuvres Zac during their trail rides. The friendship is a perfect partnership. As Shane puts it, "He [Zac] looks after me". Even a fall from Zac when he refused a jump couldn't hold Shane back, and they still regularly join the group on fox hunts and 25 mile rides. He has served on the board of directors of the Georgia Pharmacy Association and as president of the National Pharmaceutical Association as well as chairman of the board of directors of the National Pharmaceutical Association Foundation. A member of the Governor's Advisory Council on Science and Technology Development, Matthews has been chairman of the Commission on Pharmaceutical Care and of the Commission to Revise the Georgia Pharmacy Practice Act of the Georgia Pharmacy Association. He recently completed a six-year term on the American Council on Pharmaceutical Education, serving a two-year term as secretary treasurer. Matthews has written more than 100 publications and abstracts and has given more than 275 professional and scientific presentations at local, state, national and international meetings. He is a recipient of the Dean's Award from the American College of Apothecaries for outstanding service to community pharmacy practice. Dr. Matthews also has received the Wendell T. Among the most important drug classes in the treatment of Stage B through D heart failure are angiotensin-converting enzyme ACE ; inhibitors. An analysis of the major studies suggested that ACE inhibitors may reduce the risk of death, heart attack, and hospital admissions by 28% in patients with existing congestive heart failure. These agents block the effects of the renin-angiotensin-aldosterone system, which is thought to play a powerful role in the development of heart failure. By preventing the formation of an artery-constricting substance called angiotensin II, blood vessels widen and blood pressure drops, decreasing the workload of the heart. ACE inhibitors also improve heart and lung muscle function, which should be very helpful for patients with existing heart failure. For most people with existing high blood pressure and no evidence for heart failure Stage A ; , diuretics would be a better option. In an important 2003 study, diuretics achieved a lower risk for heart failure--and also stroke and angina--than an ACE inhibitor. However, another 2003 comparison study reported fewer heart attacks and lower risk for death with ACE inhibitors than with diuretics, particularly in elderly Caucasian men. More research is needed to confirm the specific benefits of each agent. In any case, ACE inhibitors are particularly important for patients with diabetes. A large study, for example, reported that diabetic patients who took these drugs had fewer heart attacks and lower all-cause mortality rates compared to those who took other anti-hypertensive agents. ACE inhibitors also may help slow progression of kidney disease, independently of their effect on blood pressure. Some experts believe, in fact, that angiotensin may be the common factor linking diabetes and high blood pressure. This natural chemical not only influences all aspects of blood pressure control but it also interferes with insulin's normal metabolic signaling. ; Brands. Respiratory status Ensure patent airway and a gastric tube in place to prevent gastric distension. ETT may be needed if respiratory distress evident although decompression of GIT may reduce respiratory compromise. Gastric distension Insert a size 8 gastric tube and fully aspirate contents. Continue to aspirate hourly and allow to free drain in between. Note the amount and colour of the aspirate. Decompression of the stomach and bowel helps to prevent partial or total obstruction of blood flow and oxygenation to the bowel. If decompression does not occur there is increased risk of bowel necrosis of the swollen intestine lying outside the abdominal cavity due to constriction from the small abdominal defect. Decompression also reduces the risk of emesis and aspiration. Thermoregulation Admit onto a warmer. The infant's legs and torso should be placed inside an impermeable bowel bag, ensuring it is secured under the infant's axilla. Use cling film if no bowel bag available. Bowel bags provide not only a sterile environment but also reduce the risk of contamination and intestinal trauma and prevent evaporative heat and fluid loss. Dry head, arms and upper torso watch for hypothermia, at risk due to large surface area of exposed bowel. Physical examination for other anomalies. Perform as thorough physical exam as you can. Repeat careful examination after surgery. Fluid volume status High risk of increased fluid loss from exposed bowel watch for symptoms of shock. Insert peripheral line and give fluid resuscitation of Normal Saline. Watch peripheral perfusion, check for metabolic acidosis and measure blood pressure frequently. The baby may need more Normal Saline if there is poor peripheral perfusion, metabolic acidosis or low blood pressure. Maintenance fluid requirements are increased to 120mls kg day or more. Glucose concentrations may need to be adjusted to prevent hyperglycaemia. Assessment of urine output is difficult until the bag is removed at surgery. Gastric compression and Intestinal compromise Bowel compromise can also occur due to incorrect positioning of the infant. Position either; supine with boundaries positioned on either side to support the herniated bowel or position on the right side so that there is no tension or kinking or the mesenteric arteries leading to vascular compromise. The Epilepsy Foundation of Northern California st enthusiastically announces the completion of our 1 annual camp for children with epilepsy. This year 25 campers from all over California met in Yosemite National Park for a one-week overnight adventure. We can proudly admit that all twenty-five campers left with smiles on their faces! In addition, eighteen counselors, two nurses, and two doctors have committed to attend next years camp. Campers participated in a number of activities including nature walks, ropes course, rock wall, swimming, horseback riding, arts and crafts, to name a few.
